Obviously up tempo wise alternating Malf Taps in 16ths.

Learn the the third 'inversion' first...
R-(r)l-r L-(l)r-l

It's a lot easier to separate the somewhat awkward accent->flam motion that way, and then work on getting them to a acceptable speed the way they're normally written (as a flam accent with the accent on the third partial) after you've become comfortable with them. It's still the same beats, you're just offsetting it by 2 partials and it makes it easier to develop the muscle memory instead of overthinking it and getting nowhere.

Another big thing is to stay relaxed, if you tense up, the flam quality and timing will end up being horrible.

There's a good Bluecoats exercise that works on this (finishing move/finishing touch or something), but it's on my computer at school... demonllama might have them, though.

I don't know about a hardest rudiment, but I always find my self practicing simple double-strokes and flams the most. When I play an excersize alone, they sound fine. But playing with someone else, it sounds completely dirty because of what all these complex cheese-da-chuda-whatevah passages are based on - my foundation is weak. Learning any rudiment once I get my basics up to par will be an adventure.

Personally I think up tempo alternating flam flams are kind of difficult to make flow whlie making them controled and having a good sound.

Out of the Original 40, id have to say flam inverts. Its really hard to learn to play them the right way and teach urself to "pop" your wrists.

Hold you back from what?

To me, the hardest rudiment is (I don't think) a registered, or whatever, rudiment. It's a herta with a flam on the end.

It's hard to play fast and clean in a line.
